Sexual Health Restoration for Men: Consultations

Dr O’Gorman has been practising Medicine since 1992. His qualifications include a Bachelor of Medicine and a Bachelor of Surgery and Fellowships in General Practice (UK, EU, and Australia) amongst many others. He has also practised in Cosmetic Medicine since 2004. Over the years he has assessed many male patients in relation to sexual health concerns and has assisted many patients with advice and prescribing.

Dr O’Gorman personally conducts all sexual health consultations. This is usually in a face to face format but consultation via phone or video can also be arranged.

During the consultation, Dr O’Gorman will enquire on your medical history, medications, any allergies, lifestyle, past surgical procedures, family history, and he will discuss your particular concerns.

The assessment will often necessitate checks such as blood pressure, height/weight ratio, screening for diabetes, cardiovascular risk assessment incl blood testing of lipid profile, hormonal assessment, and a check on prostate health.

A: Erectile Dysfunction + Peyronie’s curvature

Erectile dysfunction is the inability to obtain or maintain an erection sufficient for satisfactory sexual activity. ED can commence from any age but is common from age 40 and 50% of men experience ED by age 50.

The primary cause of ED is reduction in blood flow to the penis from changes in the circulation associated with age and arteriosclerosis. ED can also occur following spinal injuries, following prostate surgery with associated nerve damage, or as a side-effect of some medications. Sometimes there can be a psychological basis.

The first step in managing ED is to ensure the patient’s health is optimal which includes screening for diabetes and cardiovascular disease. Smoking cessation is important, management of obesity is important, as is avoidance of excess alcohol intake.

Assessment for ED commences with the consultation and investigations. An ED score will be calculated based on the IIEF*-5 questionnaire and this score will form a baseline for comparison as treatments proceed to ensure benefit is being achieved.

Treatment can include psychosexual counselling, oral medication, and injectable medication. Vacuum pumps and penile implants are also treatment options that patients may wish to explore. Often men seek a non-medication option as they may be experiencing side-effects of medication treatment or dislike the interruption to spontaneity with the timing required with medications.

At Renewed Confidence Clinic, in addition to assessment, investigation, prescribing or referring, we provide two key non-medication treatments for ED – Zimmer GentlePro shockwave therapy, and Penis PRP shot.

A1: Zimmer GentlePro Shockwave Therapy

Shockwave therapy aims to address the core cause of ED, namely enhancing the blood circulation to the penis. This can provide lasting benefits and reduce the need for the use of medication therapy.

The treatment takes approx 20 minutes, is not painful, and has no downtime. You can simply pop in during a lunch break and head back to work afterwards. It is best to attend having trimmed the pubic hair prior to the treatment. Ideally the treatment is performed twice a week for 4 – 6 weeks. Men report improvements after just one treatment but more commonly it is after 3-4 treatments before improvements are noticed and maximal results are seen at 8 weeks. Success rates of 75% to 90% of men have been reported resulting in stronger, harder, longer lasting erections. The benefits from the treatment can last many years but the treatment can be repeated at any time should the results diminish.

Benefits in addressing Peyronie’s curvature have also been observed.

A2: Penile PRP injections

Platelet rich plasma (PRP) can be used to treat both ED and Peyronie’s.

All patients will have undergone a prior consultation with investigation, assessment and having been deemed appropriate for a PRP treatment.

The treatment involves our nurse drawing off some of your blood just like having a blood test. The blood then undergoes centrifugation (spinning) allowing separation of the PRP from the red blood cells. The PRP contains multiple peptides and growth factors that help to promote healthy tissue and vasculature. This leads to enhanced blood flow and nerve sensitivity. At Renewed Confidence, we use a highly concentrating PRP system which generates much more concentrated PRP than standard tube systems.

You can expect to be in our care for approx 1 hour. The treatment is performed under local anaesthetic to keep the procedure comfortable for you. It is recommended that you attend having trimmed the genital regions prior.

Please also avoid blood thinners such as aspirin, anti-inflammatories, steroids, vitamin E, fish oil, gingko biloba etc for 3-4 days prior to your treatment. Mild swelling, tenderness, or bruising may occur. There is no downtime as such and you can return to normal activities straight away. We do recommend refraining from sexual intercourse for a couple of days however.

Most patients have 3 treatments with 4-6 weeks in between each treatment.

Benefits generally last 12-18 months and a single annual maintenance treatment is recommended. Benefits include improved longer lasting erections, prolonged sexual intercourse, improved sensation, and improved penile skin condition and appearance. Penile PRP has a success rate of approx 70%.

This treatment can also be performed as an add on to Zimmer shockwave therapy.

Intimacy Peel

The aging process takes place in the genital-perineal and adjacent areas in the same way as in the rest of the body. Because of this, loss of elasticity and firmness as well as a change in the colour of the area can be seen.

Intimate hyperpigmentation is a disorder caused by an excess formation of melanin that involves skin darkening around the anal and external genital area. In addition, the fact that melanocyte activity is greater in this area than in the rest of the body is also responsible for the high prevalence of the colour change affecting it.

It is a disorder of multifactorial origin, and the main triggering factors include:

Internal Factors

Age: aging of the genital tissue is caused by changes in the biological, biochemical and molecular mechanisms that can result in pigmentation disorders.

Genetic predisposition: the prevalence is higher in persons with high phototypes (IV, V and VI).

External Factors

Care and beauty habits: techniques such as depilation can cause abrasion and folliculitis, leading to skin inflammation that results in an increased production of the melanocytes and therefore of melanin.

Clothing habits: when the skin experiences friction, it naturally thickens (hyperkeratinisation) and darker areas appear. It often affects the inner thighs in case of frequent use of tight pants or underwear.
